The neg. lead of the battery goes to the board. The pos. lead goes to the ring of the input jack. For the LED, attach your current limiting resistor to the cathode (the flat side). That goes to the -9v supply. The anode (round side) goes to whichever lug on your 3PDT you want to use for switching the LED on and off.
