When I press bypass I hear a very loud click
I Have 5V of DC Voltage in the output of pedal.
Can You help me?

Do you mean you have 5v output in the pedal when bypassed and measuring the output jack?

Check your 3PDT wiring for shorts, especially with the LED. Also do you have a pulldown resistor installed?
