As far as I can tell- the gain and tone function as they should, but the output is extremely low, any ideas?

Looks like great guts. Have you substituted anything? Have you got an audio probe to try and trace? If you did, start with the output and work backwards. Having the schematic handy can help with that.

Sometimes, using a 10k instead of a 100k resistor or something like that can happen.
