What you are describing sounds like either one LED is in the wrong way or it is shorted to ground. Check to see if the LEDs are arranged back to back (can't tell from your pic). If you have a continuity checker, see if either LED has both ends connected to ground (which would indicate a short).

Over-drill the jack holes and put locking washers on the inside. That will let you move the jacks up toward the enclosure lid.

No need to replace if it is just scratchy. Get some contact cleaner and spray it in there. Turn the pot a dozen times and you are good to go.

This is also helpful when using new Alpha pots. The contact cleaner loosens up pots quite a bit.

The plus end on electrolytic caps always go toward the higher voltage source.
For C6, + goes to R8.
C7, + to pin1 of IC1
C17, + to R18
C20, + to R21

You can also try the volume pot at the input of the (my drawn) circuit. In that case you would use a 10k resistor in the R5 position (your schematic).

You don't need R1, R2 or R5 in this design and you may even be introducing noise through R2. Make R3 1M to 4M7 and it will work fine. Try breaboarding this schematic with the same value pot in your volume pedal and see if you get any noise. It'll only take a couple of minutes.

Have you tried connecting pin11 to ground via a 1k resistor? Pulling pin11 up to 5v might be making the rate too fast to detect. Just a guess.

Should be just under 100mA. The BTDR-2 is 60mA and the PT2399 is probably about 1/3 of that (since there are 3 PT2399 in the BTDR). So, I would guess 85-90mA typical with 100mA peak.
