You gotta wire your D.C. Input to the 1/4" input. You use green for in and yellow for out also, I've done that for several years now.

I think the pcb wouldn't be hard. i think the ring lens is harder to do or aquire.

Looks like it was 2 things: the DC jack was shorting against the case, and a ground wire not connected to the sleeve of the output jack.
