I'd reflow and and make sure you have component placement correct. Check for bridges  also.

First off, jason knows his stuff, I'd be proud to have one of his populated boards also. Etch is beautiful, simple chrome knobs will probably set it on fire, the white ones you have covers some of that awesome etch. Nicely executed.

Check your part placement? A simple build like this is easy to throw the wrong parts in to, trust me on that.

Do you have a test rig so you can disconnect the stomp? If the first board was already populated and the same thing is happening with this one, and you populated it. Also, if you have a different power source like a 9volt, give that a go as well. I wonder if the 1 spot isn't the source? Also, would the color of D5 have anything to do with it? Builds that I've seen that have an LED coming off of the PT2399, the LED is always a diffused green color.

Love the tube screamer, this is a great circuit for "that sound" I've built many of these and sold them to friends for cost. I've built a green bean with all the fancy switches, this one is mine. Just a plain ol 808. I used 4144 trannies, gave it more bite.

Replace q1 and look for shorts to ground after the base where there should be none

Yaayy another pedal to just add to my collection, I have a few commercial pedals to just collect, this is one of them.  Picked it up new for 60.00  These things are huge!!!!
