On your test rig, I'm almost positive you've got the tip and sleeve on your input jack reversed. I can't totally tell if there's something going on with the footswitch too as I can't recognize whose wiring scheme you used, but on those enclosed Switchcraft jacks, that lug on the diagonal corner is ground.
